It is the process of documenting any adverse event, risk or potential risk that has occurred or may occur in the workplace, community setting or a participant home. This includes near misses, changes to normal routine, injury, illness, abuse, neglect, or any other safety-related issue. However, it is important to note that there are two types of incident reports that disability support services workers need to be aware of and differentiate between: internal incident forms and reportable incidents.
Incident Forms
Incident forms are internal documents that disability support services workers complete when an incident occurs. These incidents may be minor and can often be dealt with quickly and efficiently by the organisation's incident management team. Examples of incidents that may require an incident form include minor falls, medication errors with no harm, or equipment malfunctions. These incidents are not required to be reported to the NDIS Quality and Safeguards Commission.
Reportable Incidents
On the other hand, reportable incidents are more serious and require immediate action and reporting to the NDIS Quality and Safeguards Commission. These incidents involve significant harm or abuse to individuals receiving disability support services or serious safety risks to staff or other individuals. It is important to note, an internal incident form will still be required in these instances as a record. Examples of reportable incidents include physical or sexual assault, serious injury, or the death of an individual receiving disability support services. NNA DSS Approach To Incident Management
Promoting a culture of safety within the disability support services sector is vital to ensure that the individuals receiving support are safe and secure.
NNA Direct Support Services (NNA DSS) uses an integrated incident reporting system to promote a culture of safety and improve incident management within the organisation.
The incident reporting system used by the NNA DSS is user-friendly digital, and directly attached to the customer record system (CRM), allowing staff to report any incidents easily and immediately. Staff can choose a category of incident, such as work health and safety or general incident, from a dropdown menu and complete the form as prompted. The system is easily accessible through smartphones, and all reports are sent directly to their managers for review.
One of the key messages that the NNA DSS promotes is that incident reporting is not about highlighting errors or wrongdoings but is an educational tool for all parties. The system is designed to recognise patterns of behaviour, track wound presentations, and detect any decline in health or changes in mood or routine that could be an indication of a larger concern. By reporting these incidents early no matter how small, risk to safety, health or environment can be minimised if not eliminated with appropriate interventions. Furthermore, early intervention can reduce future risk of more serious incidents for both participants and their support team.
NNA DSS provides training to all staff in incident reporting and how to document correctly, NNA DSS openly ask all staff to engage with their managers should they have any concerns or queries and encourages all staff to complete an incident report for any occurrence that is out of the ordinary. Even small changes in an individual's mood, routine, or physical presentation can be signs of a larger concern. It is essential to recognise and respond to these changes promptly to ensure the safety and wellbeing of everyone concerned.
